The Pediatric Advanced Care Team (PACT) supports families and children with life-threatening illnesses with palliative care that prevents,reduces, and soothes symptoms. PACT physicians work with the primary health care team to treat the underlying illness, while at the same time,striving for optimal quality of life. In close collaboration with patients, families, and the primary care team, the PACT team develops a quality of life

care plan that optimizes pain and symptom management, advocates family and health care provider communication, and coordinates inpatient,outpatient, and home care. PACT clinicians also provide consultation in inpatient, outpatient, and home settings.
